Once you’ve completed your advance directive, you can:
- Scan and upload the document to your kp.org account by visiting kp.org/advancedirective OR
- Fax the document to 1-855-889-3320 OR
- Take it to a Kaiser Permanente medical center with a HIMS department. To find a medical center with a “HIMS" department near you, you can call Member Services at 703-359-7878.
