2:12:330-20: Chet Paulsen introduction and talking about the birth of Rockcuffs, understanding Blood Flow Restriction, design of BFR cuffs. Design problems are the main cause for safety issues. Truly dissecting research and understanding improper or misrepresented studies. Hormonal response to BFR, endocrine response, cause for DOMS. Using BFR for pain reduction for up to 24 hours afterwards. 20-30: BFR reduces pain, improves strength and muscle size, improve adherence to use, uses for recovery. Ability to improve Vo2 Max levels. Importance of using BFR on both limbs. 30-40: Targeting both sides with BFR as both limbs become affected after an injury/surgery. Average person does not know their 1 Rep max, can use BFR with minimal exercises with process of tightening and loosening to have benefits. BFR can have benefits during passive range of mobilization and joint mobilizations. BFR is not just mechanical it is endocrine, metabolic hack. Hormonal release is systemic, hypothalamus will put hormones in motion. Cells become more permeable to IGF Insulin growth factor. M Torque 1 is where it makes transition from blood stream to the muscle. BFR can have benefits for slowing diseases like Parkinson's due to angiogenesis process. Importance of using BFR post injury. 40-50: Contraindications to using BFR. Safety and injury issues with BFR come from high pressure. Pressure changes when exercising, Arterial Occlusion pressure changes on muscle based on position of muscle and also from rest to exercises as blood flow increases. Venous pressure is 1/10th of arterial, veins are more compliant as they have a smaller muscle around them, hold 70% of blood volume. Venous occlusion happens between 20-70mm of pressure. Rock cuffs are rigid and equal efficiency pressure of 2 inches. Pneumatic cuffs tend to bubble and limited adequate pressure, too much pressure in the middle. Most pneumatic cuffs are shiny object tools. Understanding cuff size and efficient pressure to limb size. 50-60: Pressure doubles under cuff during exercise, more pressure is not better. Time it takes for set up and ease of putting on cuffs being a factor. Understanding measuring venous pressure, difference between lean individuals and bigger individuals and use of pressure on cuff. Reinforcing again that the pressure in the cuff when the muscle is relaxed changes when the muscle is contracted. 30-15-15-15 with the rest breaks is the aerobic capacity of the muscle. The middle part of the last set, fatigue should occur. Reps should be consistent without a pause, pause means fatigue is setting in and too much pressure. Don't need fancy equipment to tell you how tight a cuff needs to be. 60-70: Understanding proper placement and use of Rockcuffs, adjusting pressure, dosage of exercise. Arterial flow is never full occluded, do not need to fully occlude arterial flow. 18-20 minutes is max time of being occluded. Feedback and observation are best clues to understanding occlusion and fatigue levels. Lactate levels above normal for 10 minutes 3x week, hypothalamus will start it's process. BFR has to be consistent and used as a supplement to exercise not just one set of exercise or one time use during a week. 70-80: BFR is a partnering with body system response not a punishment or trick system like most training programs. Calorie consumption is important for during use of BFR. Central aminos, hydration are key for building blocks IGF, and M Torque 1 process. Protein synthesis will be optimized with BFR, hydration and central aminos. Avoid caffeine for 90 minutes beforehand using BFR routine, Have caffeine after because it is a vasoconstrictor. Following BFR want vasodilation response after Un occluding. BFR allows to do exercise with less weight and less injury due to lower resistance and focus on form and technique. Understanding 30-15-15-15 and 30-30-30 regimens. Want to keep pressure low and regimens prescribed the proper way to ensure adherence and reduce DOMS. And more……

Coaches punishing their athletes for missing practice for therapy or sports psychology. Understanding what a true coach is, coaching moment and coaching principles. 40-45: Conquering irrational fears following injury. ACLs are affecting children as young as 8 years old. 45-50: Anxiety, stress and sleep deprivation are major components contributing to injuries. The toxicity of social media and how it affects the mental space of an individual coming back from injury and on a daily basis. Social media creating a complex of perfectionism. The organized sports schedule drastically affecting time for rest and recovery for youth with schoolwork and practices. Reviewing Blood Flow Restriction (BFR) use in physical therapy for rehab and performance. Review of the science of BFR combined with the innovation and art behind treating patients for optimizing results. Combining BFR with endurance and high intensity work. 15-30 Achieving the hypoxic state for benefits with BFR. Understanding testosterone release benefits from BFR. Avoiding delaying use of BFR. Combining hydration with BFR and training. Monitoring appropriate occlusion levels and understanding individual levels of occlusion with BFR, especially using the RockCuffs brand. BFR sounds scary, but the science behind it is very safe. 30-40 BFR accelerating the ability to come back faster and accelerate the healing process, motor unit recruitment, systemic effect of blood flow restriction contributing to these processes. Diving into the quad tendon graft for ACL vs. Patella tendon graft. Reviewing biomechanics of understanding the ability to decelerate through the single leg, frontal plane control, biomechanics outweighs pure strength for return to sport function. Single leg hop test continues to be outdated and poor correlation to function. Considering bias in return to sport testing. 40-50 Fighting the tendency as a PT to score with a bias for return to play for your own patients, valuing the objective data. Training jumping with poor mechanics is not beneficial if there is no focus on control and mechanics. Curtailing the training program to target the deficits. 50-55 Trent discuses rotational strength and body control, implementing with jujitsu and combat art. DR. Should high school athletes turn back to sport between 6-9 months mark post operative ACL repair? The data is clear, earlier return, higher chance of re tearing. Understanding importance of the ability to decelerate. Not ready to return until athlete can decelerate under high force. The problem with "decel drills" being used and not testing an individual under running 100%. Interpreting the FLY 10 TEST. Looking at athlete's best vertical jump. 5-10 The single leg hop and triple leg hop test is antiquated and not practical. How physical therapy is going backwards as a profession with sports performance. Poor criteria for return to play. 10-15 Machine company pushing products and influencing rehab and training environments. Leg extensions are not as important as being implied. Plenty of functional exercises that will isolate the quads. The poor status and presentation of athletes being discharged from physical therapy. Go to exercise is the split squat and assault bike sprints! Athletes should not be in pain during rehab! 15-20 The problem with the Biodex and isolated testing as a hang the hat on metric. Considering multiple factors. Really diving into the "research" and looking at bias and self funded researching, old school "quad and hamstring" talk. 20-25 Understanding why functional movements are important for sports performance and translating into the sport. ACL multi factorial, considering the turf and the shoes. Turf and shoe companies want no responsibility for contributing to tears. Injuries still on grass, still overall issues with poor training even at professional level. Importance of unilateral hopping bounding and progressions. 10 single leg squats with 30% of bodyweight. 25-30 Importance of single leg jumping towards the mid line. Understanding female puberty contributing to injury. Gaining weight in breast size and wider hips but not enough muscle mass. Wrong shoes on wrong surfaces, Body position affecting injury in sports, field hockey vs soccer and lacrosse. Knee bent position. Problem with "all purpose" cleats. Rehab needs to get better. Approaching 4 month mark and starting to progress into strength and conditioning. 30-35 Poor programming in therapy and wasting time. Understanding growth spurts and proper progressions to establish check points and getting strong at weak links, no away around it. Plyometric progressions, able to single leg hop 5 times over a 12-inch hurdle. The ability to demonstrate "athleticism". 35-40 Power and reactivity, strength and deceleration techniques. Understanding good trainers and good physical therapists. What it takes to be in a late stage return to play setting. CSCS vs. CFSC certification. Dr. Mike and I discuss a HOT TOPIC "WHAT DOES RETURN TO PLAY ACTUALLY MEAN"? And the "VALIDITY OF THE TRIPLE HOP DISTANCE TEST AS A RETURN TO SPORT ASSESSMENT" Question asked by a mom in the "Parents of the ACL Recovery" page My daughter is 5.5 months post op, quad graft, and went for her 6 month check up today. They ended up testing her for return to play, and she passed with a score of 98%, meaning her surgery leg is almost equal to her other leg! I thought I would post videos of some of the tests that they did in hopes they will help someone. They also did a basic quad and hamstring strength test, and she was equal on those. Now we are waiting on the dr. to call to let us know what exactly this means. She is a 15 yr old ECNL soccer player and has been cleared by her PT to do everything EXCEPT contact for the past few weeks. Just curious what “return to play” actually means, as I know she will not be playing in games this early! 𝗥𝗲𝘁𝘂𝗿𝗻𝗶𝗻𝗴 𝘁𝗼 𝗦𝗽𝗼𝗿𝘁𝘀 𝗣𝗮𝗿𝘁𝗶𝗰𝗶𝗽𝗮𝘁𝗶𝗼𝗻 Goals at 9-12 months: PREPARE TO TAKE RETURN TO SPORTS TEST - Display a normal running pattern that does not increase pain, swelling, or warmth - Practice and display no hesitation or compensation strategies during agility drills (particularly when decelerating) when performed at 100% effort - Practice and display normal loading (no medial knee collapse) and soft, athletic landings from all jumps and hops - Practice and display no hesitation or compensation strategies during cutting drills (particularly when decelerating) when performed at perceived 100% effort. “Return to Play” it is necessary to differentiate between various types of return: Level 1 (10) NON contact practices-stretching, warm-up, skills and drills. No more than 1 practice per day • Return to reduced team training practice (no contact) Level 2 (10) May participate in Contact or Possesion skills/drills. No scrimmage or games. No more than 1 practice per day • Return to full (normal) team training practice (with contact) Level 3 (5) 10 minutes continuous play before resting. My 13 year old dd had ACL and Meniscus surgery on rt knee May 4th. She’s been doing great but the last couple days she says it’s painful on the inside of her knee just above her knee cap. Anyone else’s child have pain?
My daughter is 3 months post-op ACL reconstruction (quad tendon) and small meniscus repair. She has been doing great and has full flexion/extension. Lately she notices that when she fully extends her leg, that she feels a shifting sensation in her knee when she bends it again. No pain, just a weird feeling. Anyone experience this?…

1:11:400-15: Who DorsaVi is, how long they have been around and how Dr. Trent Nessler implemented research backed tests to be used with DorsaVi assessments. Joanna’s history and how she became involved with DorsaVI as well as her background as a physical therapist. 15-30: Athletic Movement Index (AMI) Battery of tests takes 15-20 minutes. What patients are appropriate for the AMI testing, what tests are performed and at what stage in rehab they are appropriate. DorsaVI provides video analysis and a report printed out with the objective data. Different athletes and positions require different treatment programs. Current objective measures in the physical therapy profession are lacking efficiency. The profession needs to adapt and move forward with more advanced objective measures. 30-45: The wearable sensors can be used for multiple positions and even outside running. Some agility drills can be completed with the sensors on. The method behind how the sensors are applied and the easy to apply system. DorsaVi can be used in multiple patient populations to analyze movement. 45-60: How the system can be used for analyzing teams and illustrate the importance of athletes requiring a stronger foundation. The difficulty behind playing multiple sports and specialization early on is detrimental towards development of adolescents. DorsaVi can assess baselines throughout growth spurts and changes in body mass and height. DorsaVi is very portable, minimally cumbersome and easy to use anywhere. The report prints in 15 minutes. Each athlete and each sport requires movements and stressors that act differently on each person which limits normative value data. Physical Therapists need to get more involved in the research and question current treatment approaches. 30-45: Blood Flow Restriction Training (BFRT) can help with recovery following surgical procedures and Trent explains his first hand experience with using BFR. ACL protocols drastically differ across regions of the country and lack standardization. Kinesiophobia can be reduced by introducing early on single limb training. The severity of deficits amongst the current adolescent sports population. My 16 y.o. son is 4 months post Surgery from an ACL and meniscus repair. His PT said it was time to ask his ortho for a sports brace fitted for activity. He went to his ortho doc who said he didn’t need a brace. He said they don’t typically fit braces for ACL repairs but only do it if it makes the patient feel better. My son is a soccer goalie, so playing without a brace sounds great. Obviously we’re not there yet. My son has been cleared to run, bike, juggle, and light passing. So, is the work now just strengthening the leg? At what point did you stop PT and then did you enlist a trainer to get your kiddo game fit?…
